Wikipedia - USAir Flight 427:

US Air Flight 427 was a scheduled flight from Chicago's O'Hare International Airport to Pittsburgh, with a final destination of West Palm Beach, Florida.  The flight crashed on Thursday, September 8, 1994, killing everyone on board.

The Boeing 737-3B7 flying the route, registered N513AU, was approaching runway 28R of Pittsburgh International Airport, located in Findlay Township, Pennsylvania.  The airport was, at the time, the largest hub for the airline.

CRASH

45-year-old Captain Peter Germano and 38-year-old First Officer Charles B. "Chuck" Emmett III piloted the aircraft.  At about 6,000 feet (1,800 m) and 6 miles (10 km) from the runway, at 7:03PM, the aircraft experienced a sudden loss of control and slammed into the ground in an 80 degree nose down postion, while banked 60 degrees to the right, and traveling at 300mph in Hopewell Township, Beaver County, near Aliquippa, Pennsylvania, killing all 127 passengers and 5 crew members.  The plane just barely missed the Aliquippa exit, which was crowded with cars full of people driving home from work.

Flight 427 has the third highest death toll of any aviation accident involving a Boeing 737-300 after the crash of Flash Airlines Flight 604 and China Southern Airlines Flight 3943.  When it occurred, it was the second deadliest accident involving a Boeing 737 and is currently the sixth deadliest.  It was also the seventh deadliest aviation disaster in the history of the United States at the time it occurred; as of 2012, it now ranks ninth.

INVESTIGATION

After the longest investigation in aviation history - more than four and a half years - the concluding statement said:

   The National Transportation Safety Board determines that the probable cause of the USAir flight 427 accident was a loss of control of the airplane resulting from the movement of the rudder surface to its blowdown limit.  The rudder surface most likely deflected in a direction opposite to that commanded by the pilots as a result of a jam of the main rudder power control unit servo valve secondary slide to the servo valve housing offset from its neutral position and overtravel of the primary slide.

The National Transportation Safety Board concluded that similar rudder problems caused the previously mysterious March 3, 1991 crash of United Airlines Flight 585, and the June 9, 1996 incident involving Eastwind Airlines Flight 517, both of which were Boeing 737s.  As a result of the investigation, pilots were warned of and trained how to deal with insufficient aileron authority at an airspeed at or less than 190 knots (218 mph, 354 km/h), formerly the usual approach speed for a Boeing 737.  Four additional channels of information-pilot rudder pedal commands-were incorporated into flight data recorders, while Boeing redesigned the rudder system on 737s and retrofitted existing craft until the affected systems could be replaced.  The United States Congress also required airlines to deal more sensitively with the families of crash victims.

US Airways 427 is no longer a valid flight number on US Airways as of January 2011.

Flight 427 was the second fatal crash in a little over two months at the company (the other being USAir Flight 1016 at Charlotte-Douglas Airport in July 1994).  The crashes contributed to the financial crisis USAir was experiencing at the time.

He was the present minister of the Church of Christ, Quinland. He retired from Peabody Coal Company and was a World War II Marine Corps veteran from July 5, 1944, until the war ended in October 1946.

Herb Maloy killed in Scioto Mishap - Pike Township Trustee Died in Traffic Accident -- The death of Herbert E. (Herb) Maloy, 45, of Rt. 3 Waverly, involved in a traffic accident last Thursday evening near Portsmouth, Ohio, has been attributed to laceration of an abdominal blood vessel and subsequent hemorrhage.  The ruling was given Monday by Dr. James F. Scott, Scioto County coroner, following an autopsy.  Maloy, a Democratic precinct committee and Jackson Township trustee, was pronounced dead on arrival at Scioto Memorial Hospital following a chain-reaction, six-car accident on U. S. 23 just north of Portsmouth near the Club Franklin. Maloy's death was the seventh to result from Scioto County traffic accidents this year.  Sheriff's Deputies Orland Leadingham and Carl Roth said the crash occurred when Maloy, who was driving a pickup truck south on U. S. 23, struck in the rear a car operated by Patricia Mullins, 20, of 818 Ninth St., Portsmouth.  The Mullins car was stopped in the inside southbound lane preparing to turn into the parking lot at Club Franklin, deputies said.  The Mullins car, crashed into the parking lot, which was jammed with cars, and struck a parked convertible owned by Harry McNeer, 1631 Carrol St.  Also damaged in the crash were cars owned by Gerald McNamer of Lucasville, William Kessinger, 2431 Scioto Trail, and Dorothy M. Dotson of Lucasville Rt. 5.  Witnesses removed Maloy from the truck immediately after the accident because of the danger of fire from a large quantity of gasoline spilled from the wrecked vehicle.  He was taken to the hospital by Emrick ambulance.  Maloy is survived by his wife, Mrs. Ruby Williams Maloy;  his father, E. A. Maloy of Waverly, Rt. 3;  two sons, Donald of Springfield and Alfred of Columbus,  a daughter, Mrs. Boyd Fontaine of Aberdeen, Md.;  a brother Paul of Waverly, Rt. 3;  six sisters, Mrs. Willard Leffler, Mrs. Wilbey Bauer Jr., and Mrs. Walter Snyder, all of Waverly, Rt. 3, Mrs. Gib Breitenbach of Waverly, Mrs. A. J. Monts [sic, Mounts] of Springfield, and Mrs. Lional Evans of Waverly, Rt. 1;  and three grandchildren.  Funeral services were held at 2 p.m. Sunday at the Omega Church of Christ in Christian Union with Rev. Marion Conkel officiating.  Burial was in the Omega Cemetery under the direction of Howe Co. Funeral Home.

George W. Godby, - Services will be conducted at 2 p.m. in Bartlett Funeral Home at St. Albans by Rev. George M. Newell.  Burial will be in Cunningham Memorial Park at St. Albans.

Mr. Godby, 67, of 601 Fifth Ave. died Friday in C&O Hospital at Huntington after a short illness.  He was an engineer and 46 year employee of the Chesapeake and Ohio Railway and member of Brotherhood of Locomotive Firemen and Engineers.

Surviving are his widow, Julia;  three sons, George E. of East Lansing, Mich., Glen S. and John W. both of St. Albans;  three daughters, Mrs. Barbara Davis of St. Albans, Mrs. Lois Chapman of Scott Depot and Mrs. Mary Starkey of Louisville, Ky.;  five brothers, Roy, Joseph and Harley all of Charleston, Bill of Dunbar and Carl of Madison;  three sisters, Mrs. Nell Tully and Mrs. Macel Tate both of Charleston and Mrs. Bess Robinson of South Charleston;  and 18 grandchildren.

Mrs. Julia Chapman Godby, 86, of 601 Fifth Ave., St. Albans, died Sunday in the home of a daughter, Mrs. Lois Chapman of 9 Pinehill Drive, Scott Depot, after a long illness.

She was a member of St. Andrews United Methodist Church, the Womens Adult Bible Class, the Sunshine Sewing Club and was a lifelong St. Albans resident.

Also surviving; daughter, Mary A. Starkey of Melbourne, Fla.;  sons, Glenn S. and John W. Godby of St. Albans; sister, Mrs. Alice Harmon of St. Albans;  brothers, Richard Johnson of DeBary, Fla., Burl Johnson of Nokomas, Fla.;  22 grandchildren;  13 great-grandchildren.

Service will be 2 p.m. Tuesday in St. Andrews United Methodist Church, St. Albans, with the Rev. John M. Geary officiating.  Burial will be in Cunningham Memorial Park.

Friends may call from 2 to 4 p.m. and 7 to 9 p.m. today at Bartlett Funeral Home, St. Albans.

Surviving: wife, Ruth;  son, Harley G. Jr. of Dunbar;  daughter, Patsy Adkins of St. Albans;  sister, Bess Robertson of South Charleston;  four grandchildren;  seven great-grandchildren.

MADISON, W.Va. -- Genevieve Eastep Price, 81, a lifelong resident of Madison, died Friday, April 27, 2001, following a short illness.

Mrs. Price was born February 14, 1920.  She was the daughter of the late Jess and Clayte Eastep and was also preceded in death by her husband, Ray C. Price, three grandchildren, Ashley and Aaron Price and Nicole Styers, a sister, Irene Bender, and a brother, Jess Eastep Jr.

Mrs. Price was a member of the Madison United Methodist Church where she formerly taught Sunday School.  She was a member of the Madison Junior Women's Club, Wildwood Garden Club, and Flowermore Garden Club.  She was a homemaker.

Survivors include three daughters, Sandra P. Casteel, Elizabethton, Tenn., Linda L. Price, Dunbar, and Mrs. David (Wilma) Styers, Camp Creek and Princeton; two sons, Lloyd D. Price, St. Albans, and the Rev. Gary R. Price, Akron, Ohio; five sisters, Patsy McCollum, E. Alton, Ill., Peggy McCormick, Ripley, Hazel McCollum and Dessie Kinder, both of Ridgeview, and Pauline Miller, St. Albans; two brothers, Harry Eastep, Boynton Beach, Fla., and Phillip E. Eastep, Ridgeview; 28 grandchildren; 29 great-grandchildren; four great-great-grandchildren; a special lifelong friend, Margaret Midkiff Buchko, Marmet; and a special nephew, Austin Price, Madison.

Visitation will be from 5 to 7 p.m. Sunday, April 29, at Handley Funeral Home in Danville.  A celebration of Mrs. Price's life will be shared by family and friends at 7 p.m.  There will be a private graveside service at Boone Memorial Park in Madison on Monday, April 30.

Katie Mae (George) Hill, 89, of Petersburg, formerly a resident of South Charleston, died Jan. 7, 2014, at Grant County Nursing Home.

She was born Feb. 10, 1924, in Olcott, to Rome and Sarah George. She was preceded in death by her husband, Freddie Hill; brothers, James and Paul George; and sisters, Genevieve Myers and Ruth Villane.

She was retired from PenneyFare Supermarket and was a member of Spring Hill Baptist Church. She was a "Rosie the Riveter," having spent World War II working at the Naval Ordnance and Armor Plant in South Charleston.

Gerry was a retired registered nurse with the Department of Health and Social Services of West Virginia.

She was a member of Charleston Baptist Temple.  Gerry moved to Charleston from Madison in 1975.

She was preceded in death by her husband, Judge K.K. Hall.

She is survived by her son, K. Keller Hall Jr. of Charleston.

The funeral service will be held at 11 a.m. on Thursday, June 26, at Charleston Baptist Temple with the Rev. Archie Snedegar officiating.   Entombment will follow in Tyler Mountain Memory Gardens, Cross Lanes.   There will be no visitation.

Surviving: wife, Judy A.; daughters, Laura A. Konrad of Missouri City, Mary Lu Duke of Marietta, Ga.; foster brother, Everett W. Stone of Hialeah, Fla.; one grandaughter; two great-grandchildren.

Born in Paradise, W.Va., she was a graduate of St. Albans High School Class of 1930, attended Marshall College, was a member of the Order of the Eastern Star, and was an avid quilter.  She was a member and deaconess of the First Baptist Church of St. Albans, where she also was a member of the Gleam Sunday School class.  She was preceded in death by her husband of 65 years, Earl A. Stone; and her brother, Roy Bowles.

Surviving are her daughters, Laura Stone Konrad and her husband, Ken, of Missouri City, Texas, and Mary Lu Stone Duke and her husband, Jack, of Marietta, Ga.;  her sister, Frances Bowles Workman Cathy and her husband, James, of Victoria, Texas;  her granddaughter, Kimberly Konrad Wilkerson and her husband, Kirk, of San Diego, Calif.;  and her great-grandchildren, Kristopher Wilkerson, serving in the United States Navy on the U.S.S. He was a retired employee of Owens-Illinois Glass Plant with 41 years of service, member of the plant's Quarter Century Club, Mahone Chapel United Methodist Church in Ashford.

Surviving: wife, Pearl Rogers;  son, Robert F. of Charlotte;  daughters, Mrs. Phyllis Caruthers of Charlotte, Miss Nina Kathleen Kirk of Lansing;  sisters, Mrs. Ethel Rucker and Miss Lelea Kirk both of Sebring, Fla., Arbutus Strom of Miami, Fla.;  six grandchildren; two great-grandchildren.

Service will be held 1 p.m. in the Mahone Chapel United Methodist Church.  Burial will be in Kirk Cemetery at Ashford.

The body is in the Leonard Johnson Funeral Home in Marmet where friends may call after noon Tuesday.

Emil A. Wheatley, 95, of Danville, passed away Dec. 29, 2006.  He was a kind loving and devoted father.

Emil was the oldest of eight siblings, three of whom are still living.  He was preceded in death by his first wife, Alice VandeLinde Wheatley and their daughters, Janet Wheatley Pfeil and Juanita Wheatley Peters.

He was a DuPont retiree with over 30 years of service, a member of Olive Branch Missionary Baptist Church and International Order of Odd Fellows.  He was a member of Boone County Genealogy Society.  He was proud of his family and their accomplishments.  He was always willing to lend a helping hand and was a kind, generous man who will be missed by all who knew him.

He is survived by his second wife, Lucille Gillenwater Price Wheatley who gave him loving attention and care during their seven years of marriage; his son, Dr. Edward Wheatley and his wife, Helena, of Charleston. Other survivors include three devoted stepdaughters, Carol Sue DeVere and her husband, Robert, Kathy Keller of Charleston, and Phyllis Casey of Huntington; six grandchildren, Brian Edward Wheatley and wife, Mary Catherine, of High Point, N.C., Elizabeth Anne Wheatley of Charleston, Susan Peters Fitzpatrick and husband, Robert, of Plainfield, Ind., Katherine Peters Martinez and husband, Al, of New Albany, Ohio, Patrick Pfeil and wife, Jessica, of Westport, Conn., Richard Pfeil of New York, N.Y.; seven great-grandchildren, Tristan, Ashley, Taylor, Elisa, Henry John, Mary Ellen, Margaret Elizabeth; five stepgrandchildren, Nicholas Keller, Benjamin DeVere and Michael Casey, all of Charleston, Melissa Holland of Maryland, Jerald Casey of Ripley; four step great-grandchildren; his sisters, Nelda Ball and Mina Ball; one brother, Phil Wheatley; and several living nieces and nephews.

Funeral service will be 2 p.m. Monday, Jan. 1 at Handley Funeral Home, Danville, with the Rev. James Sheets officiating.  Burial will follow at Memory Gardens, Madison.

In lieu of flowers, contributions can be made to Olive Branch Missionary Baptist Church, Turtle Creek.

O.V. "Dick" VandeLinde, 72, of Charleston died March 23, 1999, in Memorial Division, CAMC, after a short illness.

He was director of Erase Foundation, president of Mountain Tamers Inc. and VandeLinde Inc., co-founder and executive director of the West Virginia Surface Mining Association, a former employee of the Division of Natural Resources, a Boone County schoolteacher and also taught two years in East Africa.  He was with the U.S. Aid to Foreign Countries through West Virginia University, received a bachelor's degree in vocational agriculture and a master's degree in education from West Virginia University.  He was a Navy veteran of World War II, on the board of directors of the Fellowship Hall, Greensboro, N.C., was a member of the West Virginia Surface Mining Old-Timers, president of the founding board of CARES and a member of Village Chapel Presbyterian Church, Kanawha City.

Surviving: wife, Alice Ann VandeLinde; daughters, Barbara V. Farudi of Martinsburg, Wanda V. Mouser of Clifton, Va., Melissa R. Shamblin of Cary, N.C.; sons, David L. VandeLinde of Dunbar, Sean S. Rhodes of Los Angeles; brother, Frank VandeLinde of Brunswick, Ga.; 12 grandchildren.

Memorial service will be 11 a.m. Saturday at Village Chapel Presbyterian Church, Kanawha City, with the Rev. W.F. Mansell Jr. officiating.  The body was donated to the Human Gift Registry at West Virginia University.  Friends may call one hour prior to service at the church.  In lieu of flowers, the family suggests donations to Kanawha Valley Fellowship Home, 1107 Virginia St. E., Charleston, WV 25301, or a charity of choice.
